    摘要:

    目的:观察加味愈痛丸治疗强直性脊柱炎的疗效及对血清酪氨酸激酶/信号转导子和转录激活 子(JAK/STAT) 信号通路的影响。方法:将92 例僵直性脊柱炎患者随机分为对照组和观察组各46 例。对照 组给予甲氨蝶呤治疗,观察组在对照组基础上口服加味愈痛丸,疗程均为8 周。比较2 组治疗前后巴氏AS 疾 病活跃状况量表(BASDAI) 评分、加拿大脊柱骨关节研究协会制定的影像学评价标准(SPARCC) 评分、中 医证候积分及血清炎性因子[肿瘤坏死因子-α (TNF-α)、白细胞介素-1β (IL-1β)、白细胞介素-6(IL-6)、 巨噬细胞移动抑制因子(MIF) ],免疫球蛋白[免疫球蛋白G1(IgG1)、免疫球蛋白G2(IgG2)、免疫球蛋白 A(IgA)、免疫球蛋白M(IgM) ],疼痛介质[内皮素-1(ET-1)、前列腺素E2(PGE2)、P 物质(SP)、环氧 化酶-2(COX-2) ],JAK/STAT 信号通路[酪氨酸激酶(JAK)、磷酸化酪氨酸激酶(p-JAK)、信号转导子及转 录激活子3(STAT3)、磷酸化信号转导子及转录激活子3(p-STAT3) ]水平,并比较2 组临床疗效。结果:治 疗后及随访3、6、12 个月,观察组总有效率分别为97.7%、72.7%、52.3%、38.6%,高于同期对照组的 83.3%、40.5%、16.7%、4.8%(P<0.05)。治疗后及随访3、6、12 个月,观察组BASDAI、SPARCC、中医证 候积分均降低(P<0.05);治疗后及随访3 个月,对照组BASDAI、SPARCC 均降低(P<0.05);且治疗后及 随访3、6~12 个月观察组各评分低于同期对照组(P<0.05)。治疗后,观察组TNF-α、IL-1β、IL-6、MIF 水 平均降低(P<0.05),对照组TNF-α 水平降低(P<0.05),且观察组TNF-α、IL-1β、IL-6、MIF 水平低于对 照组(P<0.05)。治疗后,观察组IgG1、IgG2、IgA、IgM 水平均升高(P<0.05),且观察组高于对照组(P< 0.05)。治疗后,2 组ET-1、COX-2、PGE2 水平均降低(P<0.05),观察组SP 水平降低(P<0.05),且观察组 ET-1、COX-2、SP、PGE2 水平低于对照组(P<0.05)。治疗后,观察组JAK、STAT3 水平升高(P<0.05), p-JAK、p-STAT3 水平降低(P<0.05);对照组p-JAK 水平降低(P<0.05);且观察组JAK、STAT3 水平高 于对照组(P<0.05),p-JAK、p-STAT3 水平低于对照组(P<0.05)。2 组不良反应发生率比较,差异无统计 学意义(P>0.05)。结论:加味愈痛丸可有效缓解强直性脊柱炎患者的临床症状,其作用机制可能与调节血清 JAK/STAT 信号通路有关。

    Abstract:

    Abstract: Objective: To observe the therapeutic effect of Modified Yutong Pills on ankylosing spondylitis and its influence on serum Janus kinase/signal transducer and activator of transcription (JAK/ STAT) signaling pathway. Methods:A total of 92 patients with ankylosing spondylitis were randomly divided into the control group and the observation group, with 46 cases in each group. The control group was treated with Methotrexate,and the observation group was supplemented with modified Yutong Pills based on the treatment of the control group. The course of treatment was 8 weeks. The scores of Bath Ankylosing Spondylitis Disease Activity Index (BASDAI),Spondyloarthritis Research Consortium Canada (SPARCC) and traditional Chinese medicine (TCM) syndromes, and the levels of serum inflammatory factors, including tumor necrosis factor-α (TNF-α),interleukin-1β (IL-1β),interleukin-6 (IL-6) and macrophage migration inhibitory factor (MIF),immunoglobulin,including immunoglobulin G1 (IgG1),immunoglobulin G2 (IgG2), immunoglobulin A (IgA) and immunoglobulin M (IgM), pain mediators, including endothelin- 1 (ET- 1), prostaglandin E2(PGE2), substance P (SP) and cyclooxygenase- 2 (COX- 2), and JAK/STAT signaling pathways,including Janus kinase (JAK),phosphorylated JAK (p-JAK),signal transducer and activator of transcription 3 (STAT3) and phosphorylated STAT3 (p- STAT3), were compared between the two groups before and after treatment, and the clinical effects in the two groups were compared. Results: After treatment and follow- up for 3, 6 and 12 months, the total effective rate was respectively 97.7% , 72.7%,52.3% and 38.6% in the observation group,higher than that of 83.3%,40.5%,16.7% and 4.8% in the control group (P<0.05). After treatment and follow- up for 3, 6 and 12 months, the scores of BASDAI, SPARCC and TCM syndromes were decreased in the observation group (P<0.05), and after treatment and follow-up for 3 months,the scores of BASDAI and SPARCC were decreased in the control group (P<0.05);after treatment and follow-up for 3,6 and 12 months the scores in the observation group were lower than those in the control group at the same period (P<0.05). After treatment, the levels of TNF-α,IL-1β,IL-6 and MIF in the observation group were decreased (P<0.05),and the TNF-α level in the control group was decreased (P<0.05), and the levels of TNF- α, IL- 1β, IL- 6 and MIF in the observation group were lower than those in the control group (P<0.05). After treatment, the levels of IgG1,IgG2,IgA and IgM in the observation group were increased (P<0.05),and those in the observation group were higher than those in the control group (P<0.05). After treatment,the levels of ET-1,COX-2 and PGE2 in the two groups were decreased (P<0.05), and the SP level in the observation group was decreased (P<0.05);the levels of ET-1,COX-2,SP and PGE2 in the observation group were lower than those in control group (P<0.05). After treatment,the JAK and STAT3 levels in the observation group were increased (P<0.05),and the p-JAK and p-STAT3 levels were decreased (P<0.05),and the p-JAK level in the control group was decreased (P<0.05); the JAK and STAT3 levels in the observation group were higher than those in the control group (P<0.05),and the p-JAK and p-STAT3 levels were lower than those in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion: Modified Yutong Pills can effectively relieve the clinical symptoms of patients with ankylosing spondylitis,and its mechanism may be related to the regulation of serum JAK/STAT signaling pathway.
