Clinical Study on Huatan Fusui Decoction Combined with Copper Needle Scraping Therapy for Post-stroke Insomnia of Wind-Phlegm Obstructing the Collaterals Type
Abstract:Objective:To observe the clinical effect of Huatan Fusui Decoction combined with copper needle scraping therapy for post- stroke insomnia of wind- phlegm obstructing the collaterals type and its effect on cerebral hemodynamics and neurons. Methods:A total of 142 patients with post-stroke insomnia of wind-phlegm obstructing the collaterals type were divided into the control group and the treatment group according to the random number table method,with 71 cases in each group. Both groups were given basic treatment; the control group was additionally treated with copper needle scraping therapy, and the treatment group was additionally treated with Huatan Fusui Decoction based on the treatment of the control group. Both groups were treated for one month. The levels of γ- aminobutyric acid (GABA), melatonin (MT),neuron specific enolase (NSE),5-hydroxytryptamine (5-HT),ubiquitin carboxyl-terminal hydrolase L1 (UCH- L1), and brain derived neurotrophic factor (BDNF) in serum, and orexin A in plasma were compared before and after treatment in the two groups. The traditional Chinese medicine (TCM) syndrome scores and Pittsburgh Sleep Quality Index (PSQI) scores were compared between the two groups before and after treatment. The parameters of polysomnography (PSG) (sleep latency, total sleep time, and number of awakenings) and cerebral hemodynamic indicators (blood flow velocity of anterior cerebral artery, middle cerebral artery, posterior cerebral artery, and basilar artery) were compared before and after treatment between the two groups. The clinical effects and incidence of adverse reactions were evaluated in the two groups. Results:The total effective rate was 95.77% in the treatment group,higher than that of 83.10% in the control group (P<0.05). After treatment, the levels of UCH- L1 and NSE in serum and orexin A in plasma in the two groups were decreased when compared with those before treatment (P<0.05), and the levels of GABA, MT, 5- HT, and BDNF in serum were increased (P< 0.05); the levels of GABA, MT, 5- HT, and BDNF in serum in the treatment group were higher than those in the control group (P<0.05),and the levels of UCH-L1 and NSE in serum,and orexin A in plasma were lower than those in the control group (P<0.05). After treatment, the blood flow velocity of the anterior cerebral artery,middle cerebral artery,posterior artery,and basal artery in the two groups was increased when compared with that before treatment (P<0.05),and the cerebral hemodynamic indicators in the treatment group were higher than those in the control group (P<0.05). After treatment, the PSQI scores,TCM syndrome scores,and PSG parameters (sleep latency and number of awakenings) in the two groups were decreased when compared with those before treatment (P<0.05),and the PSG parameters (total sleep time) were prolonged (P<0.05);the PSQI,TCM syndrome score,and PSG parameters (sleep latency,and number of awakenings) in the treatment group were lower than those in the control group (P< 0.05), and the PSG parameter (total sleep time) was longer than that in the control group (P<0.05). No adverse reaction was observed in the two groups. Conclusion: The combination of Huatan Fushun Decoction and copper needle scraping therapy has a definite curative effect on post- stroke insomnia, which can regulate the balance of the patient's sleep- awakening cycle, nourish neurons, and improve cerebral hemodynamics and sleep quality,with high safety.
