Abstract： Objective： To observe the clinical effect of the therapy of umbrella- shape round- sharp needling combined with shoulder joint loosening on periarthritis. Methods： A total of 90 patients with periarthritis of wind-cold-damp type were randomly divided into the control group and the study group， with 45 cases in each group. The two groups were treated with shoulder joint loosening ， and the study group was additionally treated with umbrella- shape round- sharp needling. Ten days was a course of treatment， and both groups were treated for one course. Before and after treatment， the shoulder pain degree of patients was assessed by the Modified Mcgill Pain Questionnaire ， including scores of present pain intensity (PPI) ， Visual Analogue Scale (VAS) and pain rating index (PRI) ， and the shoulder mobility function was assessed by the Neer Score System. The levels of inflammatory factors，including granulocyte-macrophage colony-stimulating factor (GM-CSF)，interleukin-6 (IL-6) and transforming growth factor- β1 (TGF- β1)， and neurotransmitters， including prostaglandin E2 (PGE2)， 5- hydroxytryptamine (5- HT) and Substance P (SP)， were detected. Results： After treatment， the total effective rate in the study group was higher than that in the control group (P＜0.05). The scores of PPI，VAS and PRI in the two groups were all decreased when compared with those before treatment (P＜0.05)，and the above three scores in the study group were all lower than those in the control group (P＜0.05). The Neer shoulder joint function scores in the two groups were increased when compared with those before treatment (P＜0.05)，and the score in the study group was higher than that in the control group (P＜0.05). The levels of GM-CSF，IL-6，TGF-β1，PGE2，5-HT and SP in the two groups were all decreased when compared with those before treatment (P＜0.05)， and the above six indexes in the study group were all lower than those in the control group (P＜0.05). Conclusion：The therapy of umbrella-shape round-sharp needling combined with shoulder joint loosening for periarthritis of wind- cold- damp type can effectively reduce the pain of shoulder joint，improve the mobility function of shoulder joint，promote the recovery of shoulder joint function，and enhance the therapeutic effect.
