Abstract： Objective： To observe the clinical effect of the modified Duhuo Jisheng Decoction on assisting total knee arthroplasty for osteoarthritis. Methods：A total of 88 patients with osteoarthritis were selected and divided into the control group and the observation group according to random number table method，with 44 cases in each group. Total knee arthroplasty was performed in both groups. The control group was treated with conventional treatment， and the observation group was additionally treated with Duhuo Jisheng Decoction based on the treatment of the control group for 8 weeks. The clinical effects in the two groups were evaluated. The knee function，bone metabolism indexes and inflammatory factors were compared between the two groups before and after treatment. Results： The total effective rate was 90.91% in the observation group，higher than that of 70.45% in the control group (P＜0.05). Compared with those before treatment， the scores of each dimension of Western Ontario and McMaster University Osteoarthritis Index (WOMAC) in the two groups were significantly decreased after treatment (P＜0.05)； after treatment，the scores of each dimension in WOMAC in the observation group were lower than those in the control group (P＜0.05). Compared with those before treatment，the levels of interleukin- 1 (IL- 1)， tumor necrosis factor- α (TNF- α) and matrix metalloproteinase- 3 (MMP- 3) in the two groups were significantly decreased after treatment (P＜0.05)；after treatment，the serum levels of IL- 1，TNF-α and MMP- 3 in the observation group were lower than those in the control group (P＜0.05). After treatment， the levels of bone- specific alkaline phosphatase (BALP) and bone- γ- carboxyglutamic acid- containing protein (BGP) in the two groups were increased when compared with those before treatment， and the levels of tartrate resistant acid phosphatese- 5b (TRACP- 5b) in the two groups were decreased when compared with those before treatment (P＜0.05)； the levels of BALP and BGP in the observation group were higher，and the level of TRACP-5b in the observation group was lower than those in the control group (P＜0.05). Conclusion： Modified Duhuo Jisheng Decoction combined with total knee arthroplasty can effectively relieve the clinical symptoms of patients with osteoarthritis， reduce the body's inflammatory response，and improve bone metabolic function.
