    目的:观察安宫牛黄丸对急性脑梗死(ACI) 静脉溶栓患者炎症因子、血管内皮功能及神经功能的影响。方法:选取100 例ACI 患者,按照随机数字表法分为研究组和对照组各50 例。对照组给予尿激酶静脉溶栓治疗,研究组在对照组基础上给予安宫牛黄丸治疗。比较2 组治疗前后炎症因子、血管内皮功能,神经功能及日常生活活动能力,记录2 组不良反应。结果:治疗后,2 组高敏C-反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、降钙素原(PCT) 水平均较治疗前降低(P<0.05),研究组hs-CRP、TNF-α、IL-6、PCT 水平均低于对照组(P<0.05)。治疗后,2 组肱动脉血流介导的舒张功能(FMD)、一氧化氮(NO) 水平均较治疗前升高(P<0.05),内皮素-1 (ET-1)、血栓素A2 (TXA2) 水平均较治疗前降低(P<0.05);研究组FMD、NO 水平均高于对照组(P<0.05),ET-1、TXA2 水平均低于对照组(P<0.05)。治疗后,2 组临床神经功能缺损程度评分量表(CSS)、美国国立卫生研究院卒中量表(NIHSS) 评分均较治疗前降低(P<0.05),改良Barthel 指数(MBI) 评分均较治疗前升高(P<0.05);研究组CSS、NIHSS 评分均低于对照组(P<0.05),MBI 评分高于对照组(P<0.05)。治疗期间,2 组均无明显不良反应。结论:对ACI 静脉溶栓患者采用安宫牛黄丸治疗,可以减轻机体炎症反应、提高血管内皮功能并改善神经功能,从而提高患者的日常生活活动能力。

    Abstract:

    Abstract: Objective: To observe the effect of Angong Niuhuang pills on inflammatory factors, vascular endothelial function and neurological function in patients with acute cerebral infarction(ACI) undergoing intravenous thrombolysis. Methods: A total of 100 cases of ACI patients were selected and divided into the study group and the control group according to the random number table method, with 50 cases in each group. The control group was treated with intravenous thrombolysis with urokinase,and the study group was additionally treated with Angong Niuhuang pills based on the treatment of the control group. Inflammatory factors, vascular endothelial function and neurological function before and after treatment in the two groups were compared;adverse reactions were recorded. Results:After treatment, the levels of hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),interleukin-6 (IL- 6) and procalcitonin(PCT) in the two groups were decreased when compared with those before treatment(P<0.05); the levels of hs- CRP, TNF- α, IL- 6 and PCT in the study group were lower than those in the control group(P<0.05). After treatment,the levels of brachial artery flow-mediated dilatation (FMD) and nitric oxide(NO) were increased when compared with those before treatment(P<0.05),and the levels of endothelin-1(ET-1) and thromboxane A2(TXA2) were decreased(P<0.05);the levels of FMD and NO in the study group were higher than those in the control group(P<0.05),and the levels of ET- 1 and TXA2 were lower(P<0.05). After treatment, the scores of Chinese Stroke Scale(CSS) and National Institutes of Health Stroke Scale(NIHSS) in the two groups were decreased when compared with those before treatment(P<0.05), and the modified Barthel Index(MBI) scores were increased(P<0.05); the scores of CSS and NIHSS in the study group were lower than those in the control group(P<0.05),and the MBI score was higher(P<0.05). During treatment, there was no obvious adverse reaction in the two groups. Conclusion: The application of Angong Niuhuang pills for ACI patients undergoing intravenous thrombolysis can relieve systemic inflammatory responses,and improve vascular endothelial function and neurological function.
