    目的:基于数据挖掘分析中药内服方治疗肺癌恶性胸腔积液(MPE) 的用药规律。方法:通过检索中国知网、万方数据、维普资讯三大数据库,收集并整理2000 年1 月1 日—2020 年12 月31 日共20 年内中医药治疗肺癌MPE 疗效显著的文献,筛选出其内服方剂,通过中医传承辅助系统V2.5 的描述性分析方法,对药物的应用频数,四气、五味和归经频数进行统计;通过关联规则Apriori 分析方法,对常用药物组合、核心药物组合进行分析;通过复杂系统熵聚类的数据分析方法深度挖掘治疗肺癌MPE 的潜在新药处方。结果:共筛选出60 张处方,包含134 味中药。其中使用频率≥40%的中药共有7 味,分别为茯苓、葶苈子、白术、黄芪、大枣、甘草、桂枝。茯苓的应用频率最高,占71.67%。药物四气以寒性(260 次) 为主,其次是温性(221 次)。五味以甘味(417 次) 为主,其次是苦味(274 次),辛味(209 次)。药物归经以肺经(389 次)为主,其次是脾经(303 次)、胃经(244 次)、肾经(179 次)。设置支持度为30%,置信度为0.9 时,可获得常用药物组合共30 个,其中包含7 味中药,即茯苓、葶苈子、白术、黄芪、大枣、甘草、桂枝。设置支持度为50%,置信度为0.9 时,可获得常用药物组合共1 个,包含3 味中药,即茯苓、葶苈子、白术,分别属利水消肿药、止咳平喘药、补气药。经复杂系统熵聚类分析获得,高频中药核心组合8 组,新方4 首。结论:治疗肺癌MPE 当以补虚扶正、益气利水为主,兼以解毒抗癌、化痰散结之法,同时根据患者临床症状特点及病情程度,可配合辛温散寒化饮之法、理气健脾行水之法等治法。

    Abstract:

    Abstract: Objective: To analyze the administration rules of oral prescription of Chinese herbs for malignant pleural effusion(MPE) in lung cancer. Methods: By retrieving the three databases of China National Knowledge Internet, Wanfang Data and VIP, the literature on the significant curative effect of Chinese herbs on MPE in lung cancer in the 20 years from January 1,2000 to December 31,2020 were collected and collated, and the oral prescriptions were screened out. Through the descriptive analysis method of Traditional Chinese Medicine Inheritance Assistance System V2.5,the application frequency of the Chinese herbs and the frequency of the four qi, five favors and meridian entry were counted; the common and core Chinese herbal combinations were analyzed by Apriori analysis method; the potential new prescriptions for MPE in lung cancer were deeply mined through data analysis method of complex system entropy clustering. Results: A total of 60 prescriptions were screened, including 134 Chinese herbs. Among the Chinese herbs, a total of seven Chinese herbs had the application frequency of more than 40% , namely Poria, Semen Lepidii, Semen Descurainiae, Rhizoma Atractylodis Macrocephalae, Radix Astragali, Fructus Jujubae, Radix et Rhizoma Glycyrrhizae, and Ramulus Cinnamomi. The Poria was the most frequently used, accounting for 71.67%. The four qi of Chinese herbs were mainly cold (260 times), followed by warm(221 times). The five flavors were mainly sweet(417 times), followed by bitter(274 times) and acrid(209 times). The meridian entry was mainly lung meridian(389 times),followed by spleen meridian(303 times), stomach meridian(244 times) and kidney meridian(179 times). With the assistance degree of 30% and confidence degree of 0.9,a total of 30 common herbal combinations were obtained,including seven Chinese herbs,namely Poria,Semen Lepidii,Semen Descurainiae,Rhizoma Atractylodis Macrocephalae, Radix Astragali, Fructus Jujubae, Radix et Rhizoma Glycyrrhizae, and Ramulus Cinnamomi. With the assistance degree of 50% and confidence degree of 0.9, one common herbal combination was obtained, including three Chinese herbs, namely Poria, Semen Lepidii and Semen Descurainiae, which belonged to water- draining and swelling- dispersing medicinals, coughsuppressing and panting-calming medicinals and qi-tonifying medicinals respectively. Through the entropy cluster analysis of complex system, eight core herbal combinations with high frequency and four new prescriptions were obtained. Conclusion:The treatment of MPE in lung cancer should be mainly focused on supplementing deficiency and reinforcing healthy qi, and boosting qi and promoting urination, and combined with resolving toxin and fighting against cancer,and dissolving phlegm and dissipating masses. The treatment can be combined with other treatment methods at the same time according to the characteristics of clinical symptoms and severity of the disease,such as the method of dissipating cold and resolving fluid retention with acid-warm medicinals and the method of regulating qi and fortifying spleen to move water.
