十全大补汤辅助肠内营养支持对结直肠癌根治术患者营养状况及免疫功能的影响

    目的:观察十全大补汤辅助肠内营养支持对结直肠癌根治术后患者营养状况及免疫功能的影响。方法:选取80 例结直肠癌根治术后患者,随机分为治疗组和对照组各40 例。对照组予以肠内营养支持,治疗组在对照组的基础上予以十全大补汤内服。比较2 组中医证候积分、营养状况、免疫功能及不良反应。结果:治疗前,2 组食少纳差、腹胀腹痛、气短懒言、神疲乏力等中医证候积分比较,差异无统计学意义(P>0.05);治疗后,2 组各项证候积分均较治疗前下降(P<0.05),且治疗组各项评分均低于对照组(P<0.05)。治疗前,2 组血清白蛋白(Alb)、前白蛋白(PAB)、转铁蛋白(TRF) 比较,差异无统计学意义(P>0.05);治疗后,2 组Alb、PAB、TRF 水平均较治疗前升高(P<0.05),且治疗组Alb、PAB、TRF 水平均高于对照组(P<0.05)。治疗前,2 组免疫球蛋白-A、G、M(IgA、IgG、IgM)、T 淋巴细胞亚群(CD3+、CD4+)、CD4+/CD8+水平比较,差异无统计学意义(P>0.05);治疗后,2 组IgA、IgG、IgM、CD3+、CD4+、CD4+/CD8+水平均较治疗前升高(P<0.05),且治疗组IgA、IgG、IgM 及CD3+、CD4+、CD4+/CD8+水平均高于对照组(P<0.05)。治疗组不良反应发生率为15.00%,对照组为12.50%,2 组比较,差异无统计学意义(P>0.05)。结论:十全大补汤辅助肠内营养支持能够改善结直肠癌根治术后患者营养状况及免疫功能,且无明显增加不良反应。

    Abstract:

    Abstract:Objective:To observe the nutritional status of Shiquan Dabu tang combined with enteral nutrition support on patients after colorectal cancer radical surgery and its immune function. Methods:A total of 80 cases after colorectal cancer radical surgery were selected and randomly divided into the treatment group and the control group,40 cases in each group. The control group was given enteral nutrition support, and the treatment group was additionally given Shiquan Dabu tang based on the treatment of the control group. The scores of Chinese medicine syndrome scores,nutritional status,immune function and adverse reactions were compared between the two groups. Results: Before treatment, there was no significant difference in the scores of Chinese medicine syndromes such as poor food intake, abdominal distension, abdominal pain,shortness of breath,laziness to speak,mental fatigue and lack of strength between the two groups(P< 0.05). After treatment, the above scores of syndromes in both groups were decreased when compared with those before treatment(P<0.05),and the scores in treatment group were lower than those in control group(P<0.05). Before treatment, there was no significant difference in serum albumin(ALB),serum pre- albumin(PAB) and transferrin(TRF) between the two groups(P>0.05). After treatment,the levels of Alb,PAB and TRF in both groups were increased when compared with those before treatment(P<0.05),and the above levels in the treatment group were higher than those in the control group(P<0.05). Before treatment,there was no significant difference in Immunoglobulin-A,G,M(IgA,IgG,IgM),T lymphocyte subsets (CD3+,CD4+),CD4+/CD8+ between the two groups(P>0.05). After treatment,the levels of IgA,IgG,IgM,CD3+,CD4+, CD4+/CD8+ in the two groups were increased when compared with those before treatment(P<0.05),and the above levels in the treatment group were higher than those in the control group(P<0.05). The incidence of adverse reactions was 15.00% in the treatment group and 12.50% in the control group,there being significant difference between the two groups(P>0.05). Conclusion: Shiquan Dabu tang combined with enteral nutritional support can improve the nutritional status and immune function of patients after colorectal surgery,with no significant increase in adverse reactions.
