    目的:观察利水通窍汤对急性分泌性中耳炎大鼠血管内皮生长因子(VEGF)的表达及中耳黏膜炎症的影响。方法:健康雄性SD大鼠随机分为正常对照组、模型组、头孢克洛组、利水通窍汤低剂量组、利水通窍汤中剂量组、利水通窍汤高剂量组。除正常对照组外,其他各组大鼠采用听泡内注入内毒素制成大鼠急性分泌性中耳炎动物模型。利水通窍汤高、中、低剂量组灌胃利水通窍汤的浓度分别是1.5、1、0.5 g/mL,按每只大鼠1.05 mL/100 g灌胃;头孢克洛组灌胃头孢克洛的浓度是0.01 g/mL,按1 mL/100 g灌胃,正常对照组和模型组灌胃等量的生理盐水,连续给药7天。干预后观察各组大鼠中耳黏膜中VEGF的表达水平,同时检查中耳黏膜病变的改变。结果:与正常对照组比较,模型组大鼠中耳鼓室黏膜厚度、中耳黏膜中性粒细胞计数、中耳黏膜中VEGF的表达显著升高,差异均有统计学意义(P<0.05)。与模型组比较,利水通窍汤低、中、高剂量组和头孢克洛组大鼠中耳鼓室黏膜厚度、中耳黏膜中性粒细胞计数、中耳黏膜中VEGF的表达显著降低,差异均有统计学意义(P<0.05)。利水通窍汤各剂量组与头孢克洛组比较无统计学意义(P>0.05)。结论:利水通窍汤可以抑制中耳黏膜中VEGF的表达,减轻由内毒素引起的中耳黏膜水肿,从而抑制中耳炎症反应。

    Abstract:

    Objective:To observe the effect of Lishui Tongqiao tang on the expression level of vascular endothelial growth factor(VEGF)and the inflammation of middle ear mucosa in rats with acute secretory otitis media.Methods:Divided healthy male SD rats into six groups randomly,namely the normal control group,model group,cefaclor group,Lishui Tongqiao tang of low,middle and high dose groups.In addition to the normal control group,rats in the other groups were given injection of endotoxin in tympanic bullae to establish animal models of rats with acute secretory otitis media.The concentration of Lishui Tongqiao tang in the high,middle and low dose groups was 1.5 g/mL,1 g/mL and 0.5 g/mL respectively,and each rat in these three groups was given 1.05 mL/100 g by gavage.The concentration of cefaclor was 0.01 g/mL in the cefaclor group,and each rat in it was given 1 mL/100 g by gavage.The normal control group and the model group were given the same amount of normal saline and received continuous medication for seven days.After intervention,the expression level of VEGF in the middle ear mucosa of each group was observed,and the changes in the mucosal lesions of otitis media were examined.Results:Compared with the normal control group,the mucosa thickness of the middle ear drum,the neutrophil count in the middle ear mucosa,and the expression of VEGF in the middle ear mucosa in the model group were remarkably increased,differences being significant(P < 0.05).Compared with the model group,the mucosa thickness of the middle ear drum,the neutrophil count in the middle ear mucosa,and the expression of VEGF in the middle ear mucosa in Lishui Tongqiao tang of low,middle and high dose groups were remarkably decreased,differences being significant(P<0.05).There was no significant difference being found in the comparison of the above three items among the Lishui Tongqiao tang groups of each dose and the cefaclor group(P>0.05).Conclusion:Lishui Tongqiao tang can inhibit the expression of VEGF in the middle ear mucosa and reduce the middle ear mucosal edema caused by endotoxin,thus inhibiting the middle ear inflammatory responses.
