    目的:观察加味桃红四物汤联合高压氧治疗糖尿病下肢血管病变的临床疗效。方法:选取糖尿病 下肢血管病变患者60 例,按随机数字表法分为观察组与对照组各30 例。对照组给予高压氧治疗,观察组在对 照组的基础上给予加味桃红四物汤治疗。比较2 组临床疗效、症状体征积分、腘动脉与足背动脉血管内径和血 流量、血管内皮功能指标[一氧化氮(NO)、内皮素-1(ET-1)]变化。结果:观察组总有效率为93.33%,高 于对照组73.33%,组间比较,差异有统计学意义(P<0.05)。治疗前,2 组症状体征积分比较,差异无统计学 意义(P>0.05);治疗后,2 组症状体征积分均较治疗前降低(P<0.05),且观察组症状体征积分低于对照 组(P<0.05)。治疗前,2 组腘动脉(左侧、右侧)、足背动脉(左侧、右侧) 血管内径比较,差异无统计学 意义(P>0.05);治疗后,2 组上述各项指标均较治疗前增大(P<0.05),且观察组上述各项指标均大于对照 组(P<0.05)。治疗前,2 组腘动脉(左侧、右侧)、足背动脉(左侧、右侧) 血流量比较,差异无统计学意 义(P>0.05);治疗后,2 组上述各项指标均较治疗前增大(P<0.05),且观察组各项指标均大于对照 组(P<0.05)。治疗前,2 组血清NO、ET-1 水平比较,差异无统计学意义(P>0.05)。治疗后,2 组血清NO 水平较治疗前升高(P<0.05),ET-1 水平较治疗前降低(P<0.05);且观察组上述2 项指标改善较对照组更 显著(P<0.05)。结论:加味桃红四物汤联合高压氧治疗糖尿病下肢血管病变疗效确切,能有效缓解患者症状 体征,增加血管内径和血流量,改善血管内皮功能。

    Abstract:

    Abstract: Objective: To observe the clinical effect of modified Taohong Siwu Decoction combined with hyperbaric oxygen for diabetic lower extremity vascular disease. Methods: A total of 60 cases of patients with diabetic lower extremity vascular disease were selected and divided into the observation group and the control group according to the random number table method, with 30 cases in each group. The control group was treated with hyperbaric oxygen,and the observation group was additionally treated with modified Taohong Siwu Decoction based on the treatment of the control group. The clinical effects and the changes in scores of symptoms and signs,vascular internal diameter and blood flow of popliteal artery and dorsalis pedis artery,and vascular endothelial function including nitric oxide (NO) and endothelin- 1 (ET-1) were compared between the two groups. Results:The total effective rate was 93.33% in the observation group,higher than that of 73.33% in the control group,the difference being significant between the two groups (P<0.05). Before treatment,there was no significant difference being found in the comparison of scores of symptoms and signs between the two groups (P>0.05); after treatment, the scores of symptoms and signs in the two groups were decreased when compared with those before treatment (P< 0.05),and the scores of symptoms and signs in the observation group were lower than those in the control group (P<0.05). Before treatment,there was no significant difference being found in the comparison of the vascular internal diameter of popliteal artery (left and right) and dorsalis pedis artery (left and right) between the two groups (P>0.05); after treatment, the above indexes in the two groups were increased when compared with those before treatment (P<0.05), and the above indexes in the observation group were higher than those in the control group (P<0.05). Before treatment, there was no significant difference being found in the comparison of the blood flow of popliteal artery (left and right) and dorsalis pedis artery (left and right) between the two groups (P>0.05);after treatment,the above indexes in the two groups were increased when compared with those before treatment (P<0.05), and the above indexes in the observation group were higher than those in the control group (P<0.05). Before treatment,there was no significant difference being found in the comparison of the levels of NO and ET-1 in serum between the two groups (P>0.05). After treatment, the levels of NO in serum in the two groups were increased when compared with those before treatment (P<0.05),and the levels of ET-1 were decreased when compared with those before treatment (P<0.05);the improvement of the above two indexes in the observation group was more significant than that in the control group (P<0.05). Conclusion: Modified Taohong Siwu Decoction combined with hyperbaric oxygen for diabetic lower extremity vascular disease has a definite curative effect, and can effectively relieve symptoms and signs of patients, increase vascular internal diameter and blood flow,and improve vascular endothelial function.
