Abstract：Objective：To observe the clinical effect of Wenshen Tongluo Decoction combined with salt-separated moxibustion on idiopathic membranous nephropathy of spleen-kidney yang deficiency type complicated with renal interstitial fibrosis on the basis of routine therapy in modern medicine. Methods：A total of 90 patients with idiopathic membranous nephropathy of spleen-kidney yang deficiency type complicated with renal interstitial fibrosis treated in the Department of Nephrology，Chinese Medicine Hospital of Puyang from January 2022 to January 2024 were included as the study subjects， and divided into the control group and the study group according to the random number table method，with 45 cases in each group. The two groups were treated with routine therapy in modern medicine，and the study group was additionally treated with Wenshen Tongluo Decoction combined with salt-separated moxibustion. Both groups were treated for three months continuously. Before and after treatment， the traditional Chinese medicine syndrome scores in the two groups were evaluated；the levels of 24-hour urinary protein quantity，Nephrin，connective tissue growth factor （CTGF）， thrombospondin type-1 domain-containing 7A， B-cell activating factor of the TNF family （BAFF） ， M-type phospholipase A2 receptor， glutathione peroxidase （GSH-Px） ， reactive oxygen species （ROS）， laminin， Th17 cell percentage， Treg cell percentage were detected， and Th17/Treg ratio was calculated. Observed the occurrence of adverse reactions in the two groups. The clinical effects were compared between the two groups. Results：After treatment，the total effective rate in the study group was higher than that in the control group （P＜0.05）. The scores of edema of lower limbs，aversion to cold，soreness of waist and lack in strength，thin sloppy stool， pale face， poor appetite and abdominal distension and the total scores in the two groups were reduced when compared with those before treatment（ P＜0.05），and the above scores in the study group were lower than those in the control group （P＜0.05）. The levels of 24-hour urinary protein quantity， thrombospondin type-1 domaincontaining 7A， M-type phospholipase A2 receptor， Nephrin， laminin， CTGF， BAFF and ROS and the Th17/Treg ratio in the two groups were down-regulated when compared with those before treatment （P＜0.05）； the above nine indexes in the study group were lower than those in the control group （P＜0.05）. The GSH-Px levels in the two groups were elevated when compared with those before treatment （P＜0.05）， and the GSH-Px level in the study group was higher than that in the control group （P＜0.05）. There were no significant adverse reactions in the two groups during treatment. Conclusion： The combination use of Wenshen Tongluo Decoction and salt-separated moxibustion can improve the curative effect on idiopathic membranous nephropathy of spleen-kidney yang deficiency type complicated with renal interstitial fibrosis based on the routine therapy in modern medicine，which can regulate the inmune function， improve the glomerular filtration barrier， reduce the renal fibrosis and kidney injury and alleviate the condition with great safety.
