Meta Analysis of the Clinical Effect of Bushen Huoxue Therapy Combined with Western Medicine for Threatened Abortion Complicated with Subchorionic Hematoma
Abstract:Objective:To evaluate the clinical effect and medication rules of Bushen Huoxue therapy combined with western medicine in the treatment of threatened abortion complicated with subchorionic hematoma (SCH) through a Meta-analysis. Methods:Randomized controlled trials (RCTs) comparing the combination of Bushen Huoxue therapy and western medicine (the trial group) with western medicine alone (the control group) for the treatment of threatened abortion complicated with SCH were collected by searching databases such as CNKI,CBM,VIP,Wanfang,PubMed, Embase,and Cochrane Library. The Cochrane risk of bias tool was used for literature quality assessment,and RevMan 5.4 and Stata 18 were employed for Meta-analysis. Results:A total of 33 RCTs involving 2 608 patients were included, with 1 324 patients in the trial group and 1 284 in the control group. The western medicines used in the control group included Dydrogesterone Tablets,Progesterone Capsules/Injection,Vitamin E Soft Capsules,Chorionic Gonadotropin for Injection, Aminomethylbenzoic Acid Injection, Vitamin K Injection, Phloroglucinol Injection, Carbazochrome Tablets, and Low Molecular Weight Heparin Calcium Injection. The trial group received modified Shoutai Pills (containing at least three of the following: Taxilli Herba, Dipsaci Radix, Asini Corii Colla, and Cuscutae Semen) combined with blood-activating and stasis-resolving herbs in addition to the control group. A total of 48 Chinese herbs were involved, including 15 blood-activating and stasis-resolving herbs. The blood-activating and stasisresolving herbs with a use frequency of ≥ 5 times and their dosages were as follows:Angelicae Sinensis Radix( 5-15 g), Notoginseng Radix et Rhizoma (1.5-6 g),Salviae Miltiorrhizae Radix et Rhizoma (6-30 g),and Carbonized Typhae Pollen (10-12 g). Meta-analysis results showed that, compared with the control group, the trial group had a higher total effective rate and cure rate (P<0.001),a smaller hematoma area after treatment (P<0.001),a larger reduction in hematoma area after treatment (P<0.001), and shorter time for hematoma disappearance and vaginal bleeding cessation (P<0.001). Conclusion: The combination of Bushen Huoxue therapy and western medicine is more effective than western medicine alone in the treatment of threatened abortion complicated with SCH. Blood-activating and stasis-resolving herbs such as Angelicae Sinensis Radix, Notoginseng Radix et Rhizoma, Salviae Miltiorrhizae Radix et Rhizoma,and Carbonized Typhae Pollen are commonly used.
