Abstract: Objective: To analyze Professor LI Zongzhi's medication rules for cerebral small vessel disease (CSVD) with retinal microangiopathy using the Traditional Chinese Medicine Inheritance Computing System (V3.5). Methods: Medical records of CSVD patients with retinal microangiopathy treated by Professor LI Zongzhi from September 2020 to March 2023 were collected. Herb usage frequency, natures, flavors, and channel entries were analyzed via the System,along with association rules and cluster analyses for core prescriptions. Results: A total of 539 prescriptions involving 122 herbs were included. High-frequency herbs (>300 times) in descending order were Lophatheri Herba, Cassiae Semen, Paeoniae Radix Alba, Puerariae Lobatae Radix, Ligustri Lucidi Fructus, and Uncariae Ramulus cum Uncis. Herb natures were predominantly cold and cool,flavors sweet and bitter,with the foot-jueyin liver channel as the primary channel entry. Deficiency-replenishing and heat-eliminating herbs were frequently utilized. Association rules analysis identified 32 combinations with confidence ≥0.98. Cluster analysis yielded six core novel prescriptions, two with >100 uses: Lophatheri Herba, Cassiae Semen, Paeoniae Radix Alba, Forsythiae Fructus, Lycii Fructus, Pittosporum illicioides Makino, Puerariae Lobatae Radix and Tribuli Fructus; Cassiae Semen, Lophatheri Herba, Paeoniae Radix Alba, Ligustri Lucidi Fructus, Uncariae Ramulus cum Uncis, Puerariae Lobatae Radix, Cuscutae Semen and Dioscoreae Rhizoma. Conclusion: Professor LI Zongzhi's treatment emphasizes clearing liver and nourishing yin for CSVD with retinal microangiopathy, prioritizing clearing heat over supplementing deficiency,supplemented by clearing the heart.
