目的：观察清金化痰汤加减应用于脑卒中相关性肺炎的临床效果。方法：选择脑卒中相关性肺炎患者147 例，采用计算机随机编码法随机分为对照组74 例和观察组73 例。对照组采用常规西药进行治疗，观察组在对照组基础上加用清金化痰汤治疗。观察并比较2 组患者临床治疗效果、免疫功能、C-反应蛋白（CRP）、降钙素原（PCT）、肿瘤坏死因子-α（TNF-α） 水平和临床指标。结果：观察组总有效率为94.52%，高于对照组77.03%，差异有统计学意义（χ2=7.64，P＜0.05）。治疗前，2 组CD3+、CD8+、CD4+及CD4+/CD8+水平没有统计学差异（P＞0.05）。治疗后，2 组CD3+、CD4+及CD4+/CD8+水平较治疗前升高，CD8+较治疗前降低（P＜0.05），且观察组CD3+、CD4+及CD4+/CD8+水平高于对照组（P＜0.05）。治疗前，2 组CRP、PCT、TNF-α水平没有统计学差异（P＞0.05）。治疗后，2 组CRP、PCT、TNF-α 水平较治疗前降低（P＜0.05），且观察组CRP、PCT、TNF-α水平明显低于对照组（P＜0.05）。观察组体温恢复时间、白细胞恢复时间、住院时间明显短于对照组（P＜0.05）。结论：清金化痰汤加减应用于脑卒中相关性肺炎，可明显改善患者临床症状，提高免疫功能，缓解患者炎症和感染症状，明显缩短治疗恢复时间。
Abstract： Objective： To observe the clinical effect of modified Qingjin Huatan tang for stroke- associated pneumonia. Methods：A total of 147 cases of patients with stroke-associated pneumonia were selected and divided into the control group and the observation group according to the computer random coding method，74 and 73 cases in each group respectively. The control group was treated with routine western medicine，and the observation group was additionally treated with Qingjin Huatan tang based on the treatment of the control group. Clinical effect，immune function，the levels of C-reactive protein (CRP)，procalcitonin(PCT) and tumor necrosis factor- α(TNF- α) as well as clinical indexes in the two groups were observed and compared. Results： The total effective rate was 94.52% in the observation group， higher than that of 77.03% in the control group， the difference being significant(χ2=7.64， P＜0.05). Before treatment， there was no significant difference being found in the comparison of the levels of CD3+，CD8+，CD4+ and CD4+/CD8+ between the two groups(P＞0.05). After treatment，the levels of CD3+，CD4+ and CD4+/CD8+ in the two groups were increased when compared with those before treatment， and CD8 + was decreased(P＜0.05)； the levels of CD3 + ， CD4 + and CD4 +/CD8 + in the observation group were higher than those in the control group(P＜0.05). Before treatment， there was no significant difference being found in the comparison of the levels of CRP，PCT and TNF- α between the two groups(P＞0.05). After treatment，the levels of CRP， PCT and TNF-α in the two groups were decreased when compared with those before treatment(P＜0.05)，and the levels in the observation group were significantly lower than those in the control group(P＜0.05). The recovery time of body temperature and leukocyte as well as the length of hospital stay in the observation group were significantly shorter than those in the control group(P＜0.05). Conclusion：The application of modified Qingjin Huatan tang for stroke-associated pneumonia can significantly improve the clinical symptoms and immune function of patients， relieve the levels of inflammation and infection and significantly shorten the recovery time.
