目的:观察复方血栓通胶囊对2 型糖尿病周围神经病变(DPN) 患者的疗效。方法:将纳入研究的120 例DPN 患者随机分为对照组和观察组,每组60 例。在糖尿病基础治疗的基础上,对照组给予弥可保口服,观察组给予复方血栓通胶囊口服,治疗3 个月。观察2 组患者在治疗前后的症状改善情况、正中神经、腓总神经的感觉神经传导速度(SCV)、运动神经传导速度(MCV) 和血清中脑源性神经营养因子(BDNF)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α) 的含量变化。结果:治疗3 个月后,与治疗前比较,2 组的多伦多临床评分系统(TCSS) 评分均明显下降(P<0.05),正中神经和腓总神经的SCV 和MCV 均明显增快(P<0.05),血清BDNF 含量明显升高(P<0.05),血清IL-6、TNF-α 含量明显下降(P<0.05)。与对照组治疗后比较,观察组治疗后血清BDNF 含量较高(P<0.05),血清TNF-α 含量较低(P<0.05),腓总神经SCV 较快(P<0.05)。结论:复方血栓通胶囊能改善DPN 患者的症状与体征。其作用机制可能跟复方血栓通胶囊升高BDNF 的含量,降低IL-6、TNF-α 的含量有关。在升高BDNF 的含量和提高腓总神经SCV 方面,复方血栓通胶囊效果优于弥可保。 |

Abstract : Objective : To observe the curative effect of compound Xueshuantong capsules for patients with type 2 diabetic peripheral neuropathy(DPN). Methods:A total of 120 cases of DPN patients were randomly divided into the control
group and the observation group,60 cases in each group. Based on the basic treatment of diabetes,the control group was
additionally given the oral administration of mecobalamin, and the observation group was additionally given the oral
administration of compound Xueshuantong capsules. Both groups were treated for three months. Before and after treatment,
the changes of symptom improvement,sensory nerve conduction velocity(SCV) and motor nerve conduction velocity(MCV) of
the median nerve and common peroneal nerve,as well as contents of brain derived neurotrophic factor(BDNF),interleukin-6
(IL- 6) and tumor necrosis factor- α(TNF- α) in serum in the two groups were observed. Results: After three months of
treatment, the scores of Toronto Clinical Scoring System(TCSS) in the two groups were significantly decreased when
compared with those before treatment(P<0.05); SCV and MCV of median nerve and common peroneal nerve were
significantly increased(P<0.05);the contents of BDNF in serum were significantly increased(P<0.05);the contents of IL-6
and TNF-α in serum were significantly decreased(P<0.05). After treatment,the content of BDNF in serum in the observation
group was higher than that in the control group(P<0.05),the content of TNF-α in serum was lower(P<0.05),and the SCV
of common peroneal nerve was faster(P<0.05). Conclusion:Compound Xueshuantong capsules can improve the symptoms
and signs of DPN patients. Its mechanism may be related to the increase of BDNF content and the decrease of the contents of
IL- 6 and TNF- α. In terms of increasing the content of BDNF and the SCV of common peroneal nerve, the effect of
compound Xueshuantong capsules is better than that of mecobalamin. |
