| 目的：探讨三脏养肺汤对老年肺源性心脏病并顽固性心功能衰竭（心衰） 患者血栓前状态及心功能的影响。方法：将老年肺源性心脏病并顽固性心衰患者60例按随机数字表法分为对照组和观察组。对照组给予常规治疗，观察组在常规治疗的基础上加服三脏养肺汤治疗。比较2组患者治疗后血栓前状态、心肺功能的差异。结果：治疗前，2 组用力肺活量（FVC）、第1 秒用力呼气量（FEV1）、左室射血分数（LVEF）、收缩压（SBP） 水平比较，差异无统计学意义（P＞0.05）。治疗后，2组心肺功能较前改善，观察组改善优于对照组，差异有统计学意义（P＜0.05）。治疗前，2组血管性血友病因子（vWF）、血小板ɑ颗粒膜蛋白-140（GMP-140）、纤维蛋白原（FIB）、纤溶酶原激活物抑制剂-1（PAI-1） 水平比较，差异无统计学意义（P＞0.05）。治疗后，2组vWF、GMP-140、PAI-1、FIB较前改善，观察组改善优于对照组，差异有统计学意义（P＜0.05）。观察组早期康复治疗总有效率达90.00%，明显优于对照组（66.67%），差异有统计学意义（χ2=4.812，P＜0.05）。结论：三脏养肺汤能够明显改善老年肺源性心脏病并顽固性心衰患者血栓前状态及心功能水平，提高临床疗效。 |

| Abstract：Objective：To discuss the effect of Sanzang Yangfei tang on pre-thrombotic state and the heart function in senile patients with cor pulmonale complicated with intractable heart failure. Methods：A total of 60 cases of senile patients with cor pulmonale complicated with intractable heart failure were divided into the control group and the observation group according to the random number table method. The control group was given routine treatment，and the observation group was additionally given Sanzang Yangfei tang. The differences of pre- thrombotic state and cardiopulmonary function after treatment were compared between the two groups. Results：Before treatment，when compared forced vital capacity(FVC)， forced expiratory volume in one second(FEV1)，left ventricular ejection fraction(LVEF) and the levels of systolic blood pressure (SBP) between the two groups， there was no significance in the difference(P＞0.05). After treatment， cardiopulmonary function in the two groups was improved when compared with that before treatment， and the improvement of cardiopulmonary function in the observation group was better than that in the control group，the difference being significant (P＜0.05). Before treatment， when compared the levels of von willebrand factor(vWF)， platelet α- granule membrane protein(GMP-140)， fibrinogen(FIB) and plasminogen activator inhibitor-1(PAI-1) between the two groups ， there was no significance in the difference(P＞0.05). After treatment，the levels of vWF，GMP- 140，PAI- 1 and FIB in the two groups were improved when compared with those before treatment，and the improvement of the above levels in the observation group was better than that in the control group， the difference being significant(P＜0.05). The total effective rate of early rehabilitation therapy was 90.00% in the observation group，significantly better than that of 66.67% in the control group，the difference being significant(χ 2=4.812，P＜0.05). Conclusion：Sanzang Yangfei tang can significantly improve pre-thrombotic state and the level of the heart function in senile patients with cor pulmonale complicated with intractable heart failure，as well as improve the clinical effect. |
