| 目的：观察宣肺健脾推拿联合针灸治疗小儿过敏性鼻炎的临床疗效。方法：选取231 例过敏性鼻炎患儿，按随机数字表法分为观察组112 例和对照组119 例。2 组患儿均予以常规西医治疗，对照组在此基础上予以针灸治疗，观察组在对照组基础上予以宣肺健脾推拿治疗。比较2 组患儿治疗前后体征、症状评分、炎性因子、鼻部生理功能、维生素D 水平等变化；随访1 年，统计复发率。结果：观察组总有效率为91.96%，高于对照组73.11%，差异有统计学意义（P＜0.05）。治疗后，2 组体征、症状评分较治疗前降低，且观察组体征、症状评分低于对照组，差异均有统计学意义（P＜0.05）。治疗后，2 组鼻腔总阻力较治疗前降低，鼻黏膜纤毛传输率、鼻部通气面积较治疗前升高；且观察组鼻腔总阻力低于对照组，鼻黏膜纤毛传输率、鼻部通气面积高于对照组，差异均有统计学意义（P＜0.05）。治疗后，2 组肿瘤坏死因子-α（TNF-α）、C-反应蛋白（CRP） 和维生素D（1.25-OH-D3） 水平较治疗前降低，白细胞介素-10（IL-10） 水平较治疗前升高；且观察组TNF-α、CRP、1.25-OH-D3 水平低于对照组，IL-10 水平高于对照组；差异均有统计学意义（P＜0.05）。随访1 年，观察组复发率为8.92%，低于对照组20.16%，差异有统计学意义（P＜0.05）。结论：宣肺健脾推拿联合针灸治疗小儿过敏性鼻炎可显著改善患儿临床症状及鼻腔生理功能，有效抑制炎症因子及降低复发率，疗效确切。 |

| Abstract： Objective： To observe the clinical effect of lung- dispersing and spleen- strengthening massage combined
with acupuncture on allergic rhinitis in children. Methods：A total of 231 cases of children with allergic rhinitis were randomly
divided into the observation group(112 cases) and the control group(119 cases). Both groups received conventional western
medicine treatment； the control group was additionally treated with acupuncture， and the observation group additionally
received acupuncture combined with lung- dispersing spleen- strengthening massage. The changes of score of signs and
symptoms， inflammatory cytokines， nasal physiological function， and level of vitamin D before and after treatment were
compared between the two groups； both groups were followed up for one year， and the recurrence rate was calculated.
Results：The total effective rate was 91.96% in the observation group，higher than that of 73.11% in the control group，the
difference being significant(P＜0.05). After treatment，the scores of signs and symptoms in the two groups were respectively
decreased when compared with those before treatment，and the score in the observation group was lower than that in the
control group， differences being significant(P＜0.05). After treatment， the total nasal resistance in the two groups was
respectively decreased when compared with that before treatment， and the nasal mucociliary transmission rate and nasal
ventilation area were respectively increased when compared with those before treatment；the total nasal resistance in the
observation group was smaller than that in the control group，and the nasal mucociliary transmission rate and nasal ventilation
area were higher and larger respectively than those in the control group， differences being significant(P＜0.05). After
treatment，the levels of tumor necrosis factor- α(TNF- α)，C-reactive protein(CRP) and vitamin D(1.25-OH-D3) in the two
groups were decreased when compared with those before treatment， and the levels of interleukin- 10(IL- 10) were
increased；the levels of TNF-α，CRP and 1.25-OH-D3 in the observation group were lower than those in the control group，
and the level of IL- 10 was higher，differences being significant(P＜0.05). After one year follow- up，the recurrence rate in
the observation group was 8.92%，lower than that of 20.16% in the control group，the difference being significant(P＜0.05).
Conclusion：The therapy of lung-dispersing and spleen-strengthening massage combined with acupuncture can significantly
improve the clinical symptoms and nasal physiological function of children with allergic rhinitis，effectively inhibit inflammatory
cytokines and reduce the recurrence rate，with significant curative effect. |
