| 目的:观察中西医结合治疗类风湿性关节炎的临床疗效。方法:将100例湿热痹阻型类风湿性关节炎患者随机分为研究组和对照组各50例。对照组给予口服甲氨蝶呤片、白芍总苷胶囊、萘普生缓释胶囊治疗,研究组在对照组治疗基础上给予加味麻杏薏甘汤口服配合热痹洗方熏洗治疗。2组疗程均为3月。对2组关节疼痛数、关节肿胀数、关节压痛数、关节晨僵时间、关节功能分级等症状、体征,血沉、C-反应蛋白、类风湿因子等实验室指标以及视觉模拟标尺法(VAS)评分、DAS28评分、中医证候积分进行比较分析,评价疗效。结果:治疗后,研究组总有效率92.0%,对照组总有效率78.0%,2组比较,差异有统计学意义(P<0.05)。2组关节疼痛数、关节肿胀数、关节压痛数均较治疗前减少,关节晨僵时间较治疗前缩短,关节功能分级较治疗前改善,差异均有统计学意义(P<0.05);研究组各项指标的改善均优于对照组(P<0.05)。2组血沉、C-反应蛋白、类风湿因子均较治疗前下降(P<0.05),研究组各项指标的改善均优于对照组(P<0.05)。2组VAS评分、DAS28评分、中医症状积分均较治疗前下降(P<0.05),研究组3项分值均低于对照组(P<0.05)。研究组不良反应发生率低于对照组(P<0.05)。结论:加味麻杏薏甘汤合热痹洗方联合西药治疗湿热痹阻型类风湿性关节炎,可有效缓解患者的临床症状,取得满意的治疗效果。 |

| Objective：To observe the clinical effect of integrated Chinese and western medicine therapy for rheumatoid arthtis(RA).Methods：Selected 100 RA patients with damp-heat obstruction syndrome，and divided them into the control group and the study group being 45 cases in each.The control group was treated with oral methopterin tablets，total glucosides of paeony capsules and neproxen sustained release capsules，while the study group was given modified Maxing Yigan tang combined with Rebi washing recipe based on the treatment of the control group.Treatment of both groups lasted for three months.Compared and analyzed the amount of joint pain，joint swelling，and joint tenderness，morning stiffness time of joint，classification of joint function，body sign，erythrocyte sedimentation rate(ESR)，C-reactive protein，rheumatoid factor as well as visual analog scale(VAS)score，DAS28 score，Chinese syndrome score of both groups，and evaluated the curative effect.Results：After treatment，the total effective rate was 92.0%in the study group，and was 78.0%in the control group，the difference being significant(P＜0.05).Compared with those before treatment，amount of joint pain，joint swelling，joint tenderness of both groups was all reduced，morning stiffness time of joint was reduced，classification of joint function was improved，differences being significant(P＜0.05).Every index of the study group was better than that of the control group(P＜0.05).Levels of ESR，C-reactive protein and rheumatoid factor in both groups were all lower than those before treatment(P＜0.05)，and the improvement of the study group was better than that of the control group(P＜0.05).VAS scores，DAS28 scores，Chinese syndrome scores of both groups were all lower than those before treatment(P＜0.05)；and scores of the three indexes of the study group were all lower than that of the control group(P＜0.05).Incidence of the adverse reaction of the study group was lower than that of the control group(P＜0.05).Conclusion：Modified Maxing Yigan tang combined with Rebi washing recipe on the basis of western medicine can effectively relieve clinical symptoms of patients，and has satisfactory curative effect in treating RA patients with damp-heat obstruction. |
